***To Our Boarding Clients***
To make the drop off process easier for you and your pet, please fill out our "Boarding Information Sheet" completely before bringing your pet(s) in for boarding. Instructions are listed below on how to do this. You can also call the clinic and have a receptionist fill the sheet out for you over the phone.
You can find this form by returning to the top of this page and holding your mouse over the "Home" button; a drop down menu will appear that says "Boarding Information Sheet." Click this button and a new page will open asking you to open the "Boarding Information Sheet." Click "Open" in the pop-up download box. A Microsoft Word document will appear with feilds that are able to be filled in. In some cases, a yellow bar will appear asking to enable editing. Click either "yes" or "enable editing" whichever option appears on your version of Word.
Fill out the document and either:
1.) Print it and bring it to the clinic before the boarding appointment.
2.) Print it and mail it to us at our mailing address.

5.) You can also bring it this form at the time of boarding (however this option will not quicken the check-in process)
